Combine milk, sugar, baking soda, and salt in a wide, heavy pot.
Bring to a gentle simmer and cook uncovered for 1½–2½ hours, stirring periodically at first and nearly constantly once it darkens.
Reduce heat if it foams excessively.
When the mixture is amber, glossy, and thick enough to leave a trail across the pot bottom, remove it from the heat and add vanilla.
It will thicken further as it cools.
